Renew a Single Site License with No Changes

Requirements

Refer to Renewal Online Application webpage.


To renew your California laboratory testing license, first, log in and on your welcome screen, click the View button on the appropriate business tile. This will take you to the business' dashboard. Refer to the screenshot below:

Step 1On the business dashboard, click the Continue button on the appropriate license you are renewing. If you don't see a Continue button, please contact us and include your State ID/license number. DO NOT click "Start New Application."

Step 2Review the current Business and Site Information. Click Continue.

Step 3Click Start Renewal.

Review Business Information


Step 4On this page, you will review the business information. You can update this by clicking the Edit button as shown, or click Next if you don't have any changes:

Review Testing Site Information


Step 5On this page, you will review the testing site information. You can update this by clicking the Edit button as shown, or click Next if you don't have any changes:

Associate Sites


Step 6Read the instructions. This user manual is for a single site straight renewal; therefore, No should be selected. Then, click Next.

Are there two or more sites associated with this license? Select No for single site


Upload Documents


Step 7Upload the required documents. If you made any changes, you are required to upload a CMS 116 (PDF) federal form so we could update your information with your CLIA record.

Upload required forms LAB 182, LAB 183. The download links are on the same page.

Application Summary


Step 8Review the Application Summary. You may click the Back button if you need to correct the information, otherwise, click Next.

Application Attestation


Step 9Read the application attestation. Sign, click Accept, then click Submit.

Step 10Click the Shopping Cart button to proceed.

Step 11Select the items you want to pay, then click Proceed to Checkout.

Step 12Enter your payment details. Check the payment authorization clause. Click Submit.

Step 13Check your email provided on the payment screen to view the copy of the payment receipt. Click Close to go back to the business dashboard.

Step 14Your application will be in Pending-RenewalReview status if you made major changes. Click Close to go back to your welcome screen.
