The Radiologic Health Branch (RHB) is excited to announce to the Radiologic and Nuclear Medicine Technology Communities that X-ray and Nuclear Medicine authorizations are now electronically printable or savable by use of RHB's Online Certificate/Permit Search Tool.
In the near future, authorizations will no longer be physically printed and mailed.
If your date of birth (DOB), social security number (SSN), or individual taxpayer identification number (ITIN) are NOT in RHB's records, you will not be able to access the electronic document. Submit form 8391 (PDF) via email to RHBListC@cdph.ca.gov with "[secure]" in the subject line to update your record.
INSTRUCTIONS:
Go to RHB's Online Certificate/Permit Search Tool. Using the option to search by "certificate type" and "number," enter your authorization number (e.g., RHD 99999999) and click Search.
When your information is displayed:
- Click on Verify License - The License Verification page will be displayed.
- Complete the Captcha process.
- Enter the last 4 digits of your SSN or ITIN.
- Enter your DOB in the specified format or use the date picker.
- Click Verify
If the information is verified, the License Verification page will display “VERIFIED" and “Click to Download Document".
Click on “Click to Download Document."
Your document will then display, and you may print to your printer, or you may save the document electronically.
IMPORTANT NOTE: If you hold multiple authorizations, all authorizations will be shown on one document, except for the following limited permit categories:
- DEXA Category
- Podiatric Radiography Category
