The Chronic Disease Control Branch (CDCB) mission is to prevent and optimally manage chronic disease.

Many Californians have multiple chronic conditions which put them at greater risk for other chronic conditions, limits their ability to exercise or be a member of the workforce, and can lead to an early death. Thirty-eight percent of Californians live with at least one chronic condition.  

CDCB programs address chronic disease prevention and management with an aim to ensure all Californians experience a lifetime of wellness.

CDCB staff collaborate with local health departments, state and federal agencies, universities, non-government organizations and community-based organizations to prevent chronic disease from occurring as well as decreasing the severity and impact of a condition once it occurs.
