How to Submit an Address Change
You are required by regulations to notify Laboratory Field Services (LFS) in writing of a change of address within 30 days after the change has occurred. Submit an address change by logging into the online PERL licensing and certification system, and update your profile.
Your new address will be entered in the LFS data system. You will not receive a new license or certificate. The next time you renew your license or certificate you will receive your new license or certificate at your new address.
How to Submit a Name Change
You are required by regulations to notify LFS in writing of a change of name within 30 days after the change has occurred. You must obtain a new license or certificate showing your new name.
Send an email notice of the change of name to LFSrenewals@cdph.ca.gov. This notice should include the following information:
Submit a copy of the court document or other government-issued document that shows both your previous name and your new name.
If you would like to order a duplicate license or certificate, log in to the online PERL licensing and certification system, and select the "Reprint license" option on your user dashboard.
You will receive a new license or certificate with your new name, but the expiration date will remain the same as on your old license or certificate. The new license or certificate with your new name must be posted at your work location.
How to Order a Duplicate License or Certificate
Login to the online PERL licensing and certification system, and select the “Reprint license” option on your user dashboard. Follow the prompts to submit payment and complete your request.
