End of Life Option Act

The End of Life Option Act (EOLA) allows an adult diagnosed with a terminal disease, who meets certain qualifications, to request the aid-in-dying drugs from their attending physician. The Act requires physicians to submit specified forms and information to the California Department of Public Health (CDPH). CDPH will collect data from forms submitted by physicians. Annual reports will be made pursuant to the Act and relevant privacy requirements.

The New EOLA Application Is Now Live

The California Department of Public Health (CDPH) is pleased to announce the official launch of the newly redesigned End of Life Option Act (EOLA) application.

Here's what's new:​

  • New Login Experience
    The application now features a simplified and user-friendly login process, making access more efficient and secure.
  • Updated Look and Feel
    The interface has been refreshed with a modern design that improves navigation and usability.
  • Improved Functionality
    The new system comes with a range of enhanced features, including a user dashboard with action items and a message center.

SB 380 Implementation: Key Changes to California’s End of Life Option Act Effective January 1, 2022

Effective January 1, 2022, the required waiting period between the first and second oral request is 48 hours and the Final Attestation for Aid-in-Dying Drug form is no longer required. Additionally, the text of the Interpreter’s Declaration and Patient's Request for Aid-in-Dying Drug forms has been updated. Please refer to Senate Bill (SB) 380 to review additional changes to the EOLA.

To access the EOLA portal and complete the EOLA forms online please follow the instructions below:

  1. Click on the "EOLA Online Forms Portal" image above
  2. Select "Create an Account"
  3. Select the tile labeled "Center for Health Statistics and Informatics"
  4. Select "Get Started"
  5. You may select the form that you want to complete by selecting "Apply." The first form that is required to be entered into the EOLA Portal is the Attending Physician Checklist & Compliance Form.
