Licentiates (MD, DO, DPM, DC, PA)

The information you provide to the RHB may be disclosable under the California Public Records Act (PRA). In response to PRA requests, RHB may release the address of record for Certified Supervisor and Operators and Physician Assistant (PA) fluoroscopy permit holders. If you wish to provide RHB with an alternate address that will be released to the public release upon request, please submit form CDPH 8391 (PDF).

CDPH 8230 (PDF) - California Licentiate Supervisor & Operator Permits. Use this form for the following permits:

Fluoroscopy Supervisor and Operator Permit
Radiography Supervisor and Operator Permit
Dermatology Supervisor and Operator Permit

CDPH 8231 (PDF) - California Radiology S&O Certificate. (For Radiologists & Radiation Oncologists ONLY)
CDPH 8711 (PDF) - California Licentiate X-ray Bone Densitometry S&O Permit
CDPH 9109 (PDF) - Temporary Fluoroscopy Permit Application for Supervisors and Operators. (For licensed physicians & podiatrists only.)

Important: You are required to inform RHB of any change of name or mailing address within 30 calendar days of the change. Use form CDPH 8391 (PDF).

Application Process Overview

Complete, print, and submit the application for the desired X-ray/NMT permit or certificate via US mail.

Include any fees and/or required documentation listed on the application.
Provide an e-mail address on the application.

Within 30 days of receipt, the RHB will notify you of either of the following:

The application was acceptable and a permit/certificate was issued, or
The application was acceptable and the next steps for an examination, or
The application was not accepted and any next steps.

X-ray Permits and Certificates are issued:

Within 45 days of successful examination completion (if required), or
Within 30 days of receipt of an acceptable application that does not require an examination.
