How do you apply?
Before starting an online application, review the checklist.
NOTE: If you are
not the Owner, Shareholder, CEO, Board of Directors, Vice President, Other type of owner, make sure you select
Authorized Representative (AR) as your role. AR will
not be listed on the certificate as an owner.
Step 1: Get prepared.
- Download the New Facility Application Checklist (PDF) and refer to the appropriate certificate type.
Fee Schedule (PDF) to determine the appropriate fee. Note: The federal CLIA program will bill you separately for the CLIA certification.
Step 2: Learn how to use the new online application system.

Step 3: Create an account and log in.
*The online application system is best used with desktop computers or laptops using the latest version of your browser. We do not recommend using a tablet or a mobile phone to access your account.
