× The federal government has shut down due to the failures of the President and Congress to continue government funding. Millions of Californians receiving benefits from state programs may be impacted. For now, California’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) will continue to provide services and enroll eligible families as long as funding is available. No new federal funding to California WIC will be provided until the President and Congress take action. Families should continue to use their WIC benefits and attend their WIC appointments. This information is subject to change, so please monitor the California WIC website for updates.

Laboratory Testing Sites and Personnel Lists

Laboratory Field Services can provide listings of State licensed clinical laboratory personnel and registered clinical laboratories. These lists are compiled annually. To request a list, please follow the instructions below.

We can provide an occupational list of State licensed:

  • Clinical Laboratory Scientists
  • Phlebotomists
  • Public Health Microbiologist
  • Clinical Laboratory Directors
  • Clinical Laboratory Scientist Trainees

Building Clinical Laboratory Testing Site List

We can provide a list of the following:

  • Directors associated with laboratory testing sites
  • Owners associated with laboratory testing sites

Instructions

  1. FORM:   Complete List Request Form
  2. FEE:  Enclose $500.00 fee by check;  payable to “CDPH” (Do not send cash payment). Please include your Submission Number (provided by email after form submission) listed on your check in the memo section.  Failure to provide your submission number on the check may increase the processing time of your request by two weeks.
