Laboratory Testing Sites and Personnel Lists

Laboratory Field Services can provide listings of State licensed clinical laboratory personnel and registered clinical laboratories. These lists are compiled annually. To request a list, please follow the instructions below.

 Clinical Laboratory Personnel List

We can provide an occupational list of State licensed:

• Clinical Laboratory Scientists
• Phlebotomists
  
• Public Health Microbiologist
• Clinical Laboratory Directors
  
• Clinical Laboratory Scientist Trainees

 Clinical Laboratory Testing Site List

We can provide a list of the following:

• Directors associated with laboratory testing sites
• Owners associated with laboratory testing sites

Instructions

1. FORM:  Print and complete form LAB 192 (PDF).
   
2. FEE:  Enclose $500.00 fee. Make check payable to “CDPH” (Do not send cash payment).
3. MAIL TO:

4. ​EMAIL TO:  CDPH.LFSResearchDataUnit@cdph.ca.gov with the subject line title: "List Request."
   I​n addition to mailing the physical form, be sure to email a copy of the LAB 192 form to CDPH.LFSResearchDataUnit@cdph.ca.gov. Upon receipt of the email with the copy of the LAB 192 form, you will receive an email confirmation from our staff. If you have any questions, please email us.
