The purpose of this memo is to inform AIDS Drug Assistance Program (ADAP) enrollment workers that the ADAP Consent Form has been updated. Beginning with enrollments and re-enrollments that occur on or after May 16, 2024, previous versions of the ADAP Consent Form will no longer be accepted.
Revised Consent Form
Both the English and Spanish ADAP Consent forms have been updated with language around comprehensive health coverage navigation, the Medical Monitoring Project (MMP), and clients' right to revoke or modify the authorization of the consent form by contacting CDPH. Additionally, the ADAP Consent Forms are now valid for one year.
Comprehensive Health Coverage Navigation Services
Through the eligibility screening process, it may be determined by ADAP enrollment workers or ADAP staff that a client does not have active health coverage. Enrollment workers should advise clients that in this event, they are strongly encouraged to enroll in comprehensive health coverage (i.e., health insurance, Medicare, Medi-Cal). Enrolling in comprehensive health coverage increases access to care and improves health outcomes for people living with HIV. ADAP enrollment workers must provide clients with information about health coverage that is available to them and explain that having health care coverage is required by law. This language is found on the Health Coverage tab in the ADAP Enrollment System (AES).
Both the English and Spanish ADAP Consent Forms have been updated with language that advises ADAP clients with no insurance coverage that CDPH and its agents will coordinate comprehensive health coverage navigation services to ensure that ADAP remains the payer of last resort.
Medical Monitoring Project
To further expand information sharing, the updated ADAP Consent Forms now include a client opt-in section to participate in the Medical Monitoring Project (MMP). The MMP is an annual survey that collects information about the lived experiences of people with HIV and assists local health departments with obtaining accurate information about the selected clients. If a client chooses to opt-in, enrollment workers who also participate in MMP will be able to use AES information to contact clients.
Right to Revoke
The ADAP Consent Forms also include language informing clients of their right to modify or revoke the consent and authorization in writing.
Term of Agreement
Lastly, the updated ADAP Consent Forms shall remain in effect for one year from the date of the client's signature. Please note that this was changed from two years to one year.
