The purpose of this memo is to inform PrEP Assistance Program (PrEP-AP) enrollment workers that the PrEP-AP Consent Form has been updated. Beginning with enrollments and re-enrollments that occur on or after October 1, 2024, previous versions of the PrEP-AP Consent Form will no longer be accepted.

Revised Consent Form​

Both the English and Spanish PrEP-AP Consent Form language have been updated along with the term of agreement. The updated PrEP-AP Consent Forms shall remain in effect for one year from the date of the client's signature. Please note that this was changed from two years to one year.

The updated PrEP-AP Consent Forms are now available on the Enrollment Worker SharePoint:
