Obtaining Certified Copies of Fetal Death/Still Birth Certificates

The California Department of Public Health – Vital Records (CDPH-VR) maintains certified copies of:

Fetal Death Certificates

Certified copies of fetal death certificates are available for release to anyone who can provide sufficient information to identify a specific record.

Still-Birth Certificates

Certified copies of still birth certificates are available for release to the mother or father ONLY.

Application for a Certified Copy of Fetal Death or Still Birth Certificate (VS 12/13E) form
Como Obtener Copias Certificadas de Registros de Nacimientos Fetales y Muertes Fetales (VS 12/13E SP) form
Fetal Death Certificates: Check/Money Order for $21.00 per copy, payable to "CDPH-VR".
Still -Birth Certificates: Check/Money Order for $28.00 per copy, payable to "CDPH-VR".

If requesting a confidential copy, the confidential information is only to be included on an authorized copy when specifically requested by the mother listed on the certificate. Only the mother or father may request a certified copy of a Still Birth certificate. For Mail-In Requests only.
