Delayed or Court Order Delayed Vital Events
The California
Department of Public Health – Vital Records (CDPH-VR) receives all amendment requests by a mail
in application.
Note: If the child is under one year old and the birth has not been registered, please contact the local register in the county of birth.
Register a Delayed Birth Record
The
birth record was never registered or is not on file with the State Registrar
and proper evidence and affidavits are available.

Register a Court Order Delayed Certificate of Birth, Marriage, Death, or Fetal Death
The
vital record was never registered or is not on file with the State Registrar.
The processing time begins when
CDPH-VR receives your request. Please view our current Processing
Times and Fees. When your request is fully
reviewed, CDPH-VR will contact you if
any additional documentation or information is needed to complete your request.
