Amending a California Death or Fetal Death Record

The California Department of Public Health – Vital Records (CDPH-VR) receives all amendment requests by a mail in application.​

Amend a Death Record to Correct Personal Information​

• VS 24 - Affidavit to Amend a Death Record (PDF)

• Correct most typographical or spelling errors
• Add most information not known at the time of death
  
• Add an AKA ("also known as") for the decedent
