How to Complete Your CNA Equivalency Application Package

Follow these steps to transfer your qualifications and complete your application:

1. Determine Eligibility:

If you have completed or are currently enrolled in a Registered Nurse, Licensed Vocational Nurse, or Licensed Psychiatric Technician program, received medical training in the military, or earned a similar license from a foreign country or another U.S. state, you may qualify for the Competency Evaluation without additional training.

3. Provide Fingerprint Documentation:

California Residents: Get your fingerprints taken at a California Live Scan Agency. Submit a copy of the completed Request for Live Scan Service (BCIA 8016) (PDF) form. Use the Request for Live Scan Sample (BCIA 8016 Sample) (PDF)​ form to ensure accurate completion.
If you reside outside California, complete two fingerprint cards (FD-258) and send them with a $32 check or money order payable to “The Department of Justice.” Follow the DOJ’s instructions for completing the fingerprint card.

4. Submit Proof of Training:

  • If you are a current or recent student, provide a sealed letter from your school confirming equivalent training, including the completion of at least the “Fundamentals of Nursing” course. Ensure the letter includes your name, start and completion dates of the classes and hours or units completed.
  • If you were discharged from the military, submit a copy of your DD-214 form as a substitute for the transcript.
  • Online submission: You may have official transcripts emailed directly by your training provider or school to cnatranscripts@cdph.ca.gov​, including your first name, last name, date of birth, and phone number in the email. Only transcripts can be emailed, the application package must be submitted via U.S. Postal Mail.

5. Provide Proof of Work Experience:

  • Submit documentation (e.g., paystub or W2) showing that you provided nursing or nursing-related services under the supervision of a licensed health professional within the last two years.
  • Include a letter from the facility confirming your supervised work if you were paid through a staffing agency.
  • If you are a current student or earned your degree within the last two years, you do not need to provide proof of work.

6. Mail the Application Package:

Send your completed application package via U.S. postal mail to and/or official transcripts to:

7. Monitor Email/Mail: 

We encourage you to provide an email address when you submit your application. If an email address is provided all deficiencies will be communicated via email. If no email is provided a letter will be issued via U.S. Postal mail to communicate the items, the department needs to process your application.

8. Take the Competency Evaluation Exam:

  • Upon submitting all required documentation for equivalency, CDPH will issue information regarding the competency evaluation examination via email.
  • Allow at least 30 days for CDPH to receive your exam results from the testing vendor.
After passing the exam, maintaining a clear criminal record, and receiving the examination results, CDPH will issue your CNA certificate.
