Online Application (General Acute Care Hospitals (GACH) and Acute Psychiatric Hospitals (APH) only)

The Centralized Applications Branch (CAB) is primarily responsible for processing licensing and certification applications for health care facilities and providers. Pursuant to Health and Safety Code section 1272, CAB has up to 100 days to review and process a GAC H or APH application. CAB strives to meet or exceed expectations by conducting the review process well ahead of the timeframe.​

How to Apply

An applicant must be an authorized user to submit a completed online application to the CAB.

Prior to using the GACH/APH Online Application portal, providers must first create an online account. To create an account, go to the CDPH Portal Online Services (https://portal.cdph.ca.gov), select “Sign Up”, and complete the required information. Once your account has been created, you will be required to send an “Account Verification Code” to the email address you used to sign up. Once you enter the verification code, you will be sent a verification code to the phone number you used to sign up. After you provide the verification code, you will be redirected to the CDPH Portal Online Services Dashboard.

Once logged in, you may request access to a facility by selecting “Contact Us”, selecting “General Acute Care Hospital (GACH)/Acute Psychiatric Hospital (APH)” from the drop down menu, “Access Request”, the role you are requesting access for, Facility name (Initial Users will not have this option), and the nature of your request. Once submitted, you will receive an email notification of your request. Once approved, you may then submit an application by selecting the “Services” tab and selecting “Start” under the “GACH/APH Licensure and/or Certification Application” tile.

The GACH/Online application portal contains the required information step by step through the online system. The selected application type (Initial, CHOW, and Report of Change) identifies the required information and supporting documents needed to apply for licensing and certification. After successfully submitting an online application, the system sends a confirmation email with an Online Application ID number (tracking ID number) to the provider’s designated point of contact. Providers should use the Online Application ID number when contacting the CAB about their application. The confirmation email will also include a copy of the submitted application.

Where to Submit Payments

Payment instructions to pay by e-check or credit card:

You will receive an application fee invoice, in a separate email, for the option to pay electronically. Log into the CDPH payment portal (https://payit.cdph.ca.gov/). Enter your invoice number and exact amount due stated in your invoice and follow the instructions. The exact amount due must be entered or the payment will not be accepted.

To pay by check:

Submit the application fee listed above with a copy of this Application Fee Notice. Checks must be payable to the “Department of Public Health.” For easy identification, please note the Application ID Number in the memo area. This may be referred to as Account Number for Bill Pay Services. If this number is not listed on the check, it may delay processing of your application.
