āIntermediate Care Facility Initial Application Packet
A state license is required to operate an Intermediate Care Facility (ICF) in California, which is defined as:
An ICF is āa health facility that provides inpatient care to ambulatory or non-ambulatory patients who have recurring need for skilled nursing supervision and need supportive care, but who do not require availability of continuous skilled nursing care,ā pursuant to Health and Safety Code (HSC) section 1250(c).
To apply for an Initial License, you must complete the required application packet.

An applicant must submit a completed application packet to the Centralized Applications Branch (CAB). The provider instructions are a resource to guide you through the process. The provider checklist identifies the required forms and supporting documents needed to apply for pre-approval.
