ā€‹All Facilities Letter (AFL) Summary

This AFL reminds hospice providers of the requirement to file a verified report with OSHPD on or before March 15 of each calendar year.

Hospice providers are required to file a verified report with OSHPD, on or before March 15 of each year, for the preceding calendar year. The report must include all information requested by OSHPD, including, but not limited to, data pertaining to the age of patients, diagnostic categories of patients, and number of visits by service provided. OSHPD requires reports to be submitted via the Automated Licensing Information and Report Tracking System (ALIRTS).

For information on the data and forms OSHPD requires hospice providers to submit, please visit the OSHPD forms and instructions page available online.

Failure to file the required verified report with OSHPD may result in CDPHā€™s revoking of a providerā€™s license or denying a license renewal application.
