Transfer of Facilities to the San Jose District Office (DO)



All Facilities Letter (AFL) Summary

This AFL provides notice that the San Francisco DO is transferring the oversight responsibility of the listed facilities in Santa Clara County to the San Jose DO.


Effective October 1, 2017, the San Jose DO will be responsible for the oversight of the facilities listed below. Following the transfer, report all complaint and entity reported incidents to the San Jose DO.
