All Facilities Letter (AFL) Summary

This AFL notifies all deemed general acute care hospitals, home health agencies, ambulatory surgical centers, and hospices that effective August 1, 2017, the California Department of Public Health’s (CDPH) Licensing and Certification Program (L&C) will refer low and medium non-immediate jeopardy entity reported incidents (ERIs) to accreditation organizations (AOs) for investigation.

Per the Centers for Medicare and Medicaid Services’, Survey and Certification (S&C) letter, 14-35, State Survey Agencies must refer all ERIs for deemed general acute care hospitals, home health agencies, ambulatory surgical centers, and hospices to the respective AO for investigation.

The S&C Letter is available below (PDF):

Effective August 1, 2017, CDPH will begin referring non-immediate jeopardy, low and medium ERIs at deemed facilities to AOs for resolution. Facilities should continue to report ERIs to CDPH for triage and prioritization. Following DO determination, the DO will refer appropriate incidents for investigation.

Please keep your district office updated on your deemed status and AO affiliation.

If you have any questions regarding the content of this AFL, please contact your respective L&C district office.
