The Post Survey Satisfaction e-Evaluation replaces the current hard copy Post Survey Satisfaction Evaluations form (LC-Q1). CDPH encourages facilities to complete an evaluation, to help improve the survey process. Effective December 1, 2015, all Post Survey Satisfaction Evaluations must be submitted electronically.
