Adolescent Family Life Program (AFLP) Agreement Funding Applications (AFA)

This page announces the SFYs 2023–2024, 2024–2025 and 2025–2026 AFLP AFA process that provides allocation and contract funding updates for the California Department of Public Health, Maternal, Child and Adolescent Health (CDPH/MCAH) Division.​​​

Release of SFYs 2023–2024, 2024–2025 and 2025–2026 AFLP AFA Notification.

  • AFLP AFA Checklist
  • Agency Information Form​
  • Indirect Cost Rate Form
  • Annual Inventory
  • Subcontractor Agreement Packages
  • Certification Statement for the Use of Certified Public Funds
  • Government Agency Taxpayer ID Form
  • Attestation of Compliance with the Requirements of Title XIX Funds

June 22, 2023

Last Day to Register for an optional AFA Development and Budget Training Meeting – Optional budget meetings are available for technical assistance to assist funded agencies complete their AFA materials. If a meeting is requested, local AFLP Program and Fiscal representatives with decision making authority are required to attend. AFLP meetings will be offered via TEAMS. Meetings will be scheduled on a first-come, first-served basis between June 22–29, 2023.

July 14, 2023 AFLP AFA Packages due to MCAH. If needed, please contact your Contract Liaison (CL) for any extensions.
July17, 2023 Start of the AFLP CL/PC AFA Package Review and Approval.​​
Required Package

Required AFA documents, as listed on the AFA Checklist, were emailed to awardees. CBOs  will be provided with instructions for submitting additional contract documents. For questions or additional copies of AFA forms, please contact your CM noted on your Awardee letter.
