The California Stroke Registry/California Coverdell Programā€™s (CSR/CCP)  California Stroke Registry/California Coverdell Program (ā€‹CSR/CCP) Hospital Stroke Center Chart Audits Inter-Rater Reliability and Concordance Methodology and Documentation Year 4 (2018ā€“19) (PDF) document is intended to provide guidance to hospitals that would like to understand various methods used during retrospective chart audits for stroke patients, in particular, reporting National Quality Forum measures and other data elements to the Joint Commission. Additionally, the CSR/CCP would like to improve the consistency and quality in data reporting across all hospitals in the State.

The Alzheimerā€™s Association Healthy Brain Initiative publication Protecting the Heart and the Brain: Managing Hypertension to Reduce Risk of Cognitive Decline (PDF) addresses importance of managing heart health to reduce the risk for cognitive impairment. At the population-level, stroke doubles the chance of developing dementia. There are many mechanisms that may influence this risk ā€” including stroke type, dementia type, and underlying pathophysiology ā€” and researchers do not yet fully understand the connection between stroke and dementia. Nonetheless, since up to an estimated 90 percent of strokes are preventable, mitigating stroke risk across a population provides a promising opportunity to reduce the rates of both stroke and dementia. Hypertension management is one of the best-established stroke prevention strategies.
