The Comprehensive Perinatal Services Program (CPSP) is a Medi-Cal program that provides a wide range of culturally competent services to pregnant individuals, from conception through 60 days postpartum. In addition to standard obstetric services, patients receive enhanced services in the areas of nutrition, psychosocial and health education. ​​​​​​

​​Provider Application

The CPSP Provider Application (CDPH 4448) and Additional CPSP Practitioners (CDPH 4448A) forms have been updated. The new CDPH 4448 form includes a list of required components that the applicant must sign and attest to having collected. To apply for CPSP approval, complete, sign and submit a CDPH 4448 and CDPH 4448A (if applicable) to CPSP Provider Enrollment  .

After reviewing and approving the application and the provider's Medi-Cal enrollment status, the provider ​will be advised to download the CPSP protocols, assessments and individual care plan (ICP) templates listed below.

The protocol template must be customized to the provider site within six months of CPSP approval. Send the completed protocols via email attachment to CPSP Provider Enrollment . The assessment/ICP templates are for the staff's use when delivering CPSP services. The protocol template and assessment/ICP templates are designed to be utilized together.

Effective July 1 2023, Medi-Cal providers who wish to become CPSP enrolled providers must use the latest CPSP Provider Application (CDPH 4448) and Additional CPSP Practitioners (CDPH 4448A) forms. Applications using older forms are subject to be returned.
