Radioactive Materials (RAM) License Renewals

Pursuant to Title 17, California Code of Regulations section 30194(e), each license shall expire on the expiration date specified as a condition of the license. However, the license shall continue to be valid if a timely application for renewal is filed. An application for renewal shall be timely if filed at least 30 days prior to the expiration date. The existing license shall not expire until the department has taken final action on the timely filed application for renewal.

  • ā€‹If you wish to continue to possess/use RAM at your authorized facility location, from Sub-item 10 of your license, you must submit a full/complete license renewal application package, postmarked (or email dated) at least 30 days prior to the license expiration date. You may submit your renewal package via USPS mail or by email to SPSUGeneral@cdph.ca.govā€‹
  • If you wish to request an extension of the timeline to submit your renewal application package, you must submit a letter requesting an extension; include the reason and the proposed extension deadline. Your letter must be postmarked (or email dated) no less than 30 days prior to license expiration date to be considered for approval. Once your extension is granted, you will receive an administrative amendment from the Department allowing you to continue to use your license until the amended expiration date.
A standard license renewal application package must contain all the information as required for a new license application. Licensing Guidance, checklists and forms you will need are available on our RAM Licensing pageā€‹, then scroll down to Forms, Guides and Checklists. Select the appropriate application(s), etc., for your radioactive materials program type.

From the Departmentā€™s Licensing Guidance, Checklists, and Forms pertaining to your license, i.e., medical, industrial radiography, service provider, you must provide:

  1. ā€‹ā€‹ā€‹ All the information indicated in items 1 through 14, as listed on the renewal application form (RH 2000 (PDF, 1MB) for medical licensing, RH 2050 (PDF)ā€‹ for non-medical licensing). Mark the renewal check box on the form. Include signature, title, and date on page 2; AND

  2. The renewal application attachments (instructions are located in licensing guidance/checklists, described above).

Additional information may be required when the renewal application is in the review process. Failure to provide the items/details indicated on your licensing guidance/checklist/forms will result in delays processing your application.

Annual License Fees must be paid prior to the license expiration date or your renewal application will not be processed.
