Consultation with the Mycobacterial & Parasitic Diseases Reference Unit is required prior to submission of samples.  Approval by CDC may be required.

Form LAB 416 (PDF) is required for submission of samples

Specimens that are acceptable will be determined upon consultation

Storage and preservation dependent on test request.  Refer to CDC Test Information available on the CDC website.


Transport medium is dependent on test request.

Sample/Specimen Labeling

Test subject to CLIA regulations require two patient identifiers on the specimen container and the test requisition

Shipping Instructions which Include Specimen Handling Requirements

Ship using appropriate DOT/IATA approved shipping procedures
