Health Plan Users
Health plans (HPs), independent practice associations (IPAs), and medical groups (MPs) can submit member lists to CAIR and receive back matched patient immunization histories to help them complete their HEDIS (Healthcare Effectiveness Data and Information Set) measures.
How to Enroll
Complete the data sharing agreement and secure file transfer access request forms below and send to Kavitha Perumal (kavitha.perumal@cdph.ca.gov) to enroll in the HEDIS process. Format and submit your files according to the HEDIS Patient Match Specifications document below.
Representatives of HPs, IPAs, and MPs can also be given read-only access to CAIR that allows individual patient lookup. To request a CAIR read-only user account, contact Michael Powell (michael.powell@cdph.ca.gov). If you are interested in patient match reports from Healthy Futures/RIDE in the greater San Joaquin valley, contact the Local User Support Contact in those regions.
Note to HEDIS Requestors about File Size
As of March 16, 2020, CAIR no longer processes patient input files with more than one million rows. To further reduce processing times, please reduce the size of input files as much as possible. One way to reduce file size is to remove any records known to not have a match in CAIR (e.g., a patientāās maiden or alias name). ā
