This All County Letter serves to provide information on updates that the California Department of Public Health ā Vital Records (CDPH-VR) made in May 2022, to the Certificate of Live Birth, VS 10D form.
Please see the following list identifying the updates:
ā¢ The header above fields 27D & 27E was changed:
o From āOTHER TERMINATIONS - EXCLUDE INDUCED ABORTIONSā
o To āOTHER PREGNANCY LOSSES - EXCLUDE INDUCED ABORTIONSā
ā¢ Field 27F was changed:
o From āDATE OF LAST OTHER TERMINATION ā MM/CCYYā
o To āDATE OF LAST OTHER PREGNANCY LOSS ā MM/CCYYā
ā¢ The privacy notification was expanded and moved to the back of the form.
Local registrars should provide the blank VS 10D forms to non-birthing hospitals in their district as needed. CDPH-VR recommends that all local jurisdictions maintain a small supply of paper VS 10D forms at all times to prevent additional delays in registering these births. Any of the previous versions of the paper VS 10D forms should not be used and must be confidentially destroyed.
To request blank VS 10D forms, local registrars should complete the enclosed VS 140e Forms Request and submit it to the CDPH-VR Stockroom by emailing it to
vsforms@cdph.ca.gov āor by faxing it to (916) 324-0122 for processing.
