The California Department of Public Health – Vital Records (CDPH-VR) is pleased to announce the release of a revised Local Registrar's Transmittal Report (VS 106e). The form has been revised to ensure more accurate reporting and tracking of the vital records transmitted to CDPH-VR from the local county offices.
The following fields have been added or revised:
- Type of Event
- Number of Boxes/Packages in Shipment
- Excluded LRNs and Reasons LRNs Not Included in Shipment
- Notes
- CDPH-VR Mailing Address
- Instructions
Enclosed please find a PDF fillable copy of the revised VS 106e and a sample copy of the completed form. Counties may continue to use the current version of the VS 106 form (rev. 03/08) at this time. However, effective January 1, 2025, local county offices should use the revised VS 106e form (rev. 11/2024) for the transmittal of records to CDPH-VR to help ensure accurate and detailed reporting and tracking of records received. The VS 106e form may be photocopied or printed if manual completion of the form is preferred. The electronic version of the VS 106e form will also be available to order from the CDPH-VR stockroom. As a reminder, when transmitting vital records to CDPH-VR, please ensure that the records are packaged and sent securely to avoid any damages. To ensure proper delivery, please clearly label or type the CDPH-VR mailing address and indicate the box number for each package if sending multiple boxes or packages (e.g. box 1 of 2; 2 of 2). CDPH-VR highly recommends using tracked shipping methods when sending records to CDPH-VR to allow accurate tracking and confirmation of delivery.
