The purpose of this memo is to inform AIDS Drug Assistance Program (ADAP) Enrollment Workers (EW) that their assistance is needed in collecting updated ADAP Consent Forms from a portion of their ADAP clients.
Background
In September of 2024, ADAP Enrollment Workers were informed in Management Memorandum 2024-18: Updated ADAP Consent Form and Clarification Regarding Initiating a Re-enrollment Over the Phone that the ADAP Consent Form (PDF) was updated and valid for two years. The updated ADAP Consent Form is to remain in effect for two years from the date of the client's signature. Please note that this was changed from one year to two years because depending on the circumstances, some clients are granted a little more than one year of eligibility.
Outreach
For enrolled clients who a) submitted the version of the ADAP Consent Form that is valid for one year and b) have an eligibility period that is greater than one year, ADAP advisors have begun reaching out to them before the next re-enrollment to obtain consent from a client over the phone for the duration of their eligibility. ADAP advisors have been successful in reaching some clients but need assistance from ADAP EWs in contacting clients who ADAP advisors have been unable to reach.
ADAP advisors will begin providing ADAP EWs a list of clients they have been unable to reach via email and/or phone. ADAP EWs are to reach out to the list of clients and read the ADAP Consent Form over the phone, complete the Phone/Consent Attestation form (PDF), and upload the completed Phone/Consent Attestation Form to the client's profile in the ADAP Enrollment System.
Once the Phone/Consent Attestation form has been uploaded to the client's profile, please send a secure message to your ADAP advisor in order for the ADAP advisor to accurately track which clients still remain.
If an ADAP advisor or ADAP Enrollment Worker is unable to retrieve a two-year consent form before the previous consent form expires, the ADAP advisor will reduce the client's ADAP eligibility so that it ends one year from when the one-year consent form was signed.
Please note: the Phone/Consent Attestation form is to only be used for the select clients needing to update their consent form. ADAP EWs will be contacted with specific client ID numbers to reach out to in this case. The Phone/Consent Attestation does not replace the ADAP Consent Form during re-enrollment.
