Records (Blue Card)

• California School Immunization Record (PDF) (CSIR/Blue Card/CDPH 286)
  Print copies or contact your local health department to ask about ordering free printed copies.
  

Computerized School Immunization Records

• Schools, school districts, and pre-kindergarten (child care or preschool) facilities may computerize their immunization record-keeping systems. The computerized systems must contain at a minimum the information outlined in regulation section 6070. 

School and Child Care Roste​​r Loo​kup (SCRL) Tool​

•  Use the School and Child Care Roster Lookup (SCRL) Tool offered through
  CAIR Hub to find out students' vaccine coverage information in schools. Designed for school personnel tasked with ensuring enrolled students are up-to-date in their immunizations required for school, this tool quickly returns the immunization status for each student found in the California Immunization Registry (CAIR).
