Obtaining Certified Copies of Birth Certificates

The California Department of Public Health – Vital Records (CDPH-VR) maintains a permanent, public record of every birth that has occurred in California since July 1905.

Mail I​​n Request

​​Please submit the following for a mail in request:

• Application for Certified Copy of Birth Record (VS 111) form

• Cómo Obtener Copias Certificadas de Registros de Nacimiento​ (VS 111SP) form

• Notarized Sworn Statement (VS 20) form

• Located within the application

• Only required if requesting an authorized copy
  

• Check/Money Order for $29.00 per copy, payable to "CDPH-VR"

If requesting a birth certificate that contains the confidential information, please write "Confidential Copy" on the VS 111 application form or to request a copy of a sealed record, please mail in a CDPH 9126 application.  For Mail-In Requests only.

Copies may be obtained in person at the county office where the birth was registered. Please view our County Registrar and Recorder directory to request a copy of a birth record from the county office.
