​​Mammography Medical Physicists FAQs

1. ​Where can I find the requirements for authorization as a mammography physicist?

Please allow up to 30 calendar days from receipt of an acceptable​ application.

​3. How long is a Mammography Medical Physicist authorization valid for?

​Three years.

4. How do I submit my application?

  • You can submit your initial or renewal application along with the supporting documents via email to RHBRMT@cdph.ca.gov , or
  • ​You can submit by mail to the: Registration Unit, California Department of Public Health, Radiologic Health Branch, MS 7610, P.O. Box 997414, Sacramento, CA 95899-7414.

5. Do I have to pay fees for a Mammography Medical Physicist authorization?

​No. You are not currently required to pay fees.

6. Must I submit a complete physicist report sample with my initial application or does the summary suffice?

​No, you must send the entire report that has been supervised by a currently authorized California MQA physicist.

7. I lost my authorization letter. How do I request a copy of my letter?

​Please request a copy from RHBRMT@cdph.ca.gov

8. My MQA authorization has lapsed. What do I need to do to renew?

Please contact RHBRMT@cdph.ca.gov for assistance.

9. I am applying from out of state. Are there special requirements for initial authorization?

Pursuant to Title 17, California Code of Regulations (17 CCR) section 30315.52(a)(4), you must have performed a survey of at least one facility and a total of at least ten mammography systems under the direct supervision of a currently authorized California MQA physicist.

10. Can I use my survey reports from out-of-state to count toward the survey total requirement?

​​​You may use survey reports for any machine that you evaluated. However, if you are submitting surveys under direct supervision of another medical physicist, then that physicist must be a California-approved physicist who has renewed their authorization pursuant to 17 CCR 30315.60.
