NOTICE: The California Department of Public Health, Office of Health Equity Is excited to announce the intent to award for an Additional Native American Implementation Pilot Project, Sonoma County Indian Health Project, Inc. - Sonoma County, as part of Phase II of the California Reducing Disparities Project (CRDP).  

ANNOUNCING: Release of the Native American Implementation Pilot Projects
In Summer 2016, the department was unsuccessful in awarding Native American Capacity Building Pilot Project contracts due to the lack of applicants. In order to secure new Community Defined Evidence Programs (CDEPs) that meet the department’s needs and as specified by the CRDP project, the department has revised the solicitation for the implementation phase to target CDEPs that serve the Native American population in California’s rural communities and/or serve tribal communities that have historically resided in California as specified in this solicitation.

Key Dates - The deadline for proposal submissions is November 11, 2016.  The proposal must be received by CDPH no later than 5 pm. Please see page 17 of the solicitation for all key dates.

Native American: CDPH did not receive any applications for the Native American Capacity Building Pilot Project Solicitation

 The Pilot Projects are the central component to Phase II of CRDP.  The primary goal of these projects is to validate community-defined evidence practices through rigorous evaluation.

 *Only two Capacity Building Pilot Project applications were submitted and these organizations met or exceeded the minimum scoring requirements.

    CRDP Strategic Planning Workgroup (SPW) Population Reports 

    Each of the five SPWs developed a Population Report that included recommendations for reducing disparities and removing barriers to accessing programs and services, along with an inventory of community-defined promising practices that could support efforts to reduce disparities.  Below are links to the SPWs' website and the Population Reports.
