Sierra County Prioritizes Older Adult Health

Sierra County, located in the Greater Sierra Sacramento (GSS) region, shared that they were able to complete their community health assessment (CHA) through the FoPH f​unding. While completing the CHA, the local health jurisdiction (LHJ) discovered that a large portion of the county's population is over the age of 55. The funding helped to facilitate town halls meetings within the community to dive deeper into the county’s population needs.

The county was able to develop a Senior Nursing Outreach Program and assign a part-time nurse II to perform detailed assessments on seniors in the community to identify potential barriers to staying in one's home safely. Through the California Strengthening Public Health Initiative (CASPHI) funding, the Senior Outreach Nursing Program has helped further health care quality work within Sierra County’s underserved population.  ​
