 Birth Certificate Pamphlets

Affidavit to Amend a Birth Record 
Correct spelling/typographical errors on the birth certificate, including to the sex field. Add an "AKA" (also known as) to a parent's name after a parent name change.

Amend a person's name on their birth certificate after a name change.

The Adoption Process
Register an adoption, obtain copies of adoption records, or obtain non-identifying information for birth parents and/or locate biological family members.

Supplemental Name Report – Birth
Add child's name to blank field(s) when at least one parent is available to sign the form.

Delayed Registration of Birth
Register a California birth when the birth was not registered within the first year of birth or a certified copy of the original birth certificate is not obtainable.

Court Order Delayed Certificates
Register a birth if the birth was not registered or a certified copy of the original birth certificate is not obtainable.
