Health Care Facilities
Online Application (General Acute Care Hospitals (GACH) and Acute Psyciatric Hospitals (APH) only)

The Centralized Applications Branch (CAB) is primarily responsible for processing licensing and certification applications for health care facilities and providers. CAB has implemented a new GACH/APH online application system to assist providers with the application process. An online application submission is required for any of the following:

  • Initial (new) facility or agency
  • Change of Ownership (CHOW)
  • Report of Change 

CAB processes applications in the order in which they are received. Processing times for applications and other Report of Changes vary widely due to complexity of application or reported change. The GACH/APH online application system will notify applicants with contact information once an analyst is assigned to process the application.

Per AFL 19-41, please be advised that as of July 2, 2020, CAB will ONLY accept GACH/APH online applications and will no longer accept and/or process GACH/APH paper applications received after July 1, 2020.  Refer to the Online GACH/APH Application web page for additional information or contact CABHospitals@cdph.ca.gov.

 *Exceptions: Elective Percutaneous Coronary Intervention (ePCI), Sterile Compounding, and Change of Management Company (CHMC) applications are only processed as paper applications until further notice.

How to Apply

An applicant must be an authorized user to submit a completed online application to the CAB.

Prior to using the GACH/APH Online Application providers must first create an online account. To create an account, go to the CDPH Certificates, Licenses, Permits, and Registrations Portal, select "Create an Account," and complete the required information. Once your account has been created, you will receive an email confirmation and may then use your username and password to login to the online portal. After logging into the online portal, you may start an online application by selecting "Apply Now" under "Apply for Facility Licensure" on the "Let's Get Started" page.

The GACH/APH online application contains the required information step by step through the online system. The selected application type (Initial, CHOW, and Report of Change) identifies the required information and supporting documents needed to apply for licensing and certification. After successfully submitting an online application, the system sends a confirmation email with an Online Application ID number (tracking ID number) to the provider's designated point of contact. Providers should use the Online Application ID number when contacting CAB about their application. The confirmation email will also include a copy of the submitted application. At this time, online payments are not available. Only business checks, cashier's check, or money orders made payable to the "California Department of Public Health" will be accepted.  Please submit applicable payments and a copy of the online application Thank You page directly to CAB.

To avoid duplicate applications, if you have already submitted a paper licensing application to CAB, please do not submit an application online.

Where to Submit Payments

Submit applicable payments for complete online applications to the CAB at the address listed below.  Do not send any payments to the local district office.
