​Background

The Centers for Medicare and Medicaid Services (CMS) may impose federal penalties against a SNF when it is not in compliance with one or more Medicare and Medicaid participation requirements. A portion of the funds are returned to the states where the CMPs are imposed. These funds are reinvested via a grant process to support activities that benefit SNF residents by improving their quality of life.

These activities may include:

• Resident protection and relocation assistance - Support for residents when facilities close, are decertified, or downsize, including time-limited expenses for relocating residents to home and community-based settings or alternative facilities pursuant to agreements with state Medicaid agencies
• Consumer engagement initiatives - Programs that engage residents, families, and consumers to improve the quality of care in facilities
• Facility improvement and training - Technical assistance and training programs to implement quality assurance and performance improvement initiatives
• Mental and Behavioral Health - Projects that focus on delivering evidence-based behavioral health services tailored to nursing home residents' unique needs.
• Workforce Enhancement - Projects that strengthen the nursing home workforce through improved competency, education, and training, culture change, and professional development for frontline nursing home staff, including Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and Licensed Vocational Nurses (LVNs).
  

As of April 1, 2026, CDPH's available funds for new CMP projects are $20,642,100. CDPH highly encourages SNFs and NFs to apply for CMP grants to benefit residents.​

How to Apply for CMP Funds

Applicants must complete a CMPRP Application Form and a CMPRP Application Budget Spreadsheet to be considered for CMP funding. Application materials should include details such as the target audience, intended outcome, deliverables, sustainability, and replicability of the project.

Applicants may download the current CMPRP Application Form, CMPRP Budget Spreadsheet, and CMPRP Application Handbook on the CMS CMPRP website by selecting the CMPRP Application Resources (ZIP) in the Downloads section.

Application Review

CDPH typically reviews applications within 30 business days of receipt, subject to change based on volume of applications received. Projects will be assessed based on:

• Current CMPRP application form and budget spreadsheet submitted
• Eligibility of the proposed project
• Benefit to residents of SNFs
• Proposed metrics of outcomes, replicability, budget and distribution of funds
• Targeted population: diversity, geographic locations, consideration of ADA needs
• Whether the proposal exceeds (and does not duplicate) current regulatory requirements for SNFs
  

If approved, CDPH will submit the application within 30 business days of receipt to CMS for review and signatory acceptance, subject to change based on quality and volume of applications received. Following an approval by CMS, CDPH will initiate the contract process with approved applicants.
