A medical home is not a place. A medical home is an approach to providing comprehensive high-quality care by partnering with families. It is the system in which and the method with which care is provided. The medical home model of care is the gold standard of care – the best way that care can be provided.

Medical home is a key priority for CDPH/MCAH and is highlighted as a required National Performance Measure by the Health Resources and Services Administration (HRSA) Title V Block Grant program. CDPH/MCAH is the recipient of California's Title V block grant.
CDPH is starting a new pilot project in collaboration with the California Perinatal Quality Care Collaborative (CPQCC) and their High Risk Infant Follow Up Clinic to learn more about patient, family, provider and community care experiences in relation to the medical home model for children 0-3 years of age.
