Medical Home

A medical home is not a place.  A medical home is an approach to providing comprehensive high-quality care by partnering with families.  It is the system in which and the method with which care is provided.  The medical home model of care is the gold standard of care – the best way that care can be provided. 

The American Academy of Pediatrics outlines seven components of the medical home system:

• Accessible - Care is easy for the child and the family to obtain.
  
• Family-centered - Famly is recognized as the primary caregiver and partner in case.
  
• Comprehensive - Preventaitive, primary and speciality care are provided.
  
• Continuous - Care from infance through young adulthood, including transition to adult care.
  
• Cultur​ally Effective - Respect for the child and family's culture, language and beliefts.
  
• Coordinated - Care plan is created and comunicated in partnership with the family.
  
• Compassionate - Genuine concern for the well-being of the child and family.
  

Medical home is a key priority​ for CDPH/MCAH and is highlighted as a required National Performance Measure by the Health Resources and Services Administration (HRSA) Title V Block Grant program.  CDPH/MCAH is the recipient of California's Title V block grant.
