The following application forms and documents are organized by the facility type. In addition to the listed documents, a check made payable to the “California Department of Public Health” for the applicable licensing fee must accompany the required forms before your application can be processed. Please submit all documents to the Centralized Applications Unit and not your local District Office as indicated on the checklists. The checklists are undergoing revisions at this time and the website will be updated as soon as these revisions are made.
