1What does a FACE Worksite Investigation consist of?

The FACE investigator visits the incident site, photographs the area, and discusses the incident with witnesses.  Employers are asked questions about the worksite safety program and events leading up to the worker's death. All answers remain anonymous and completely confidential. FACE investigators do not evaluate compliance with occupational safety and health standards or determine fault – our purpose is to learn from previous incidents and educate employers/employees about how to avoid the same outcome.

2How do you report a fatality?

  • To report a workplace fatality, call the Cal/OSHA enforcement unit district office closest to the workplace where the incident occurred, or call (800) 475-4019.
  • Fatalities may also be reported to the FACE investigator 24 hours a day by calling 1-800-851-2138
