California Fatality Assessment and Control Evaluation Program (FACE) – Worker Fatality Alert Archive

Worker fatality alerts feature preliminary recommendations from ongoing FACE investigations, and are posted much sooner than full-length investigation reports.  Alerts include timely prevention tips that can help workers and employers avoid similar incidents.  Once the investigation report has been published, alerts are archived on this page, along with a link to the full report.
