How to order a list of California licensed Clinical Laboratory Scientists or California Clinical Laboratories records

Clinical Laboratory Scientists list:

To order this list, which is compiled annually by the department, you must call Laboratory Field Services at one of the following numbers: (510) 620-3800.  Please leave your name, mailing address or fax number.   A form will be faxed or mailed which allows you to set the criteria used to create the list. Return this completed form to the following address:

You may order this list in one to four outputs. The following is a brief description and cost. 

1. Address Listing-computer list of names, address and license type, cost  - $150.00
2. 1-Up Avery Labels-One time use sticky labels, cost - $150.00
3. Cheshire Labels-Computer form with names, addresses in format to copy on label paper, cost - $150.00
4. CD, self-explanatory, cost - $500.00

Clinical Laboratories records: 

Laboratory Field Services can provide address listing and mailing labels of state licensed and CLIA certified clinical laboratories records in hard paper copies (records are not available in diskettes). The cost of each list is $150.00.

The address listing and mailing labels contains the name of the clinical laboratories, addresses and license numbers (no telephone number) and can be sorted by name, county, license number and zip code. These records can be purchased and made available within two weeks from date the request is received.
