Information About Application for Cytotechnologist License

  • Complete the application online at https://secure.cpshr.us/cltreg/ and pay the non-refundable application fee.
  • Request the release of your ASCP verification to the California Department of Public Health, Laboratory Field Services. You can do this online by visiting the ASCP website at http://www.ascp.org/Board-of-Certification/Verification-of-Certification and clicking on “Request Verification of Certification for California, Florida, Hawaii, Montana, or Tennessee by Email.” Online verifications are sent within 24 hours. 

    You can also mail your request with a check for $15.00 to ASCP/Board of Registry, 3335 Eagle Way, Chicago, IL 60678-1033. Verifications are processed and e-mailed from ASCP within 7-10 business days.

  • Provide a copy of your training certificate from a cytology training program accredited or approved by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or its equivalent. The training certificate should be sent with the next item.
  • Once you have completed your online application, print, sign, and mail the attestation page and cytology training certificate to:

Applications will not be processed without a signed attestation page. All applications will be evaluated in the order of date of receipt of the application and supporting documents.

Official transcripts must be sent directly from the college/university to LFS at the above address. Transcripts must show a bachelor’s or higher degree posted. 

All applicants who have attended a non-United States educational institution MUST have a copy of their official transcript sent to AACRAO (www.aacrao.org) for evaluation and request a copy to be sent to LFS. LFS only accepts evaluations from AACRAO.
