Information About Application for Cytotechnologist License
Complete the application online at https://secure.cps.ca.gov/cltreg/ and pay the non-refundable application fee.
- Please mail your request authorizing the release of your ASCP verification to California (provide email addresses of judy.schlosser@cdph.ca.gov and lfspersonnel@cdph.ca.gov) with a check for $15.00 to ASCP/Board of Registry, 3335 Eagle Way, Chicago, IL 60678-1033. Verifications are processed and e-mailed from our office within 7-10 business days.
Or, go to National Student Clearing House (NSC) website http://www.studentclearinghouse.org and request verification of certification by e-mail for $13.00 payable by credit card. E-mail a copy of your Transaction ID number from NSC to verification@ascp.org your request and authorization to have your verification e-mailed to California. These requests are processed and e-mailed from the ASCP office within 2-3 business days.
- Provide a copy of your diploma from a Cytology Training Program Accredited or approved by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) or it’s equivalent should be sent with the next item.
- Print, sign, and mail the attestation page, ASCP verification and Cytology Training diploma upon completion of online application to:

Applications will not be processed without signed attestation. All applications will be evaluated upon receipt of the application and supporting documents.
Official transcripts must be sent directly from college/university to LFS (above address). Transcripts must show that at least a Bachelor’s degree posted.
All applicants who have attended a non-United States educational institution MUST have a copy of their official transcript sent to AACRAO (approved evaluation agency) www.aacrao.org for evaluation and request a copy to be sent to LFS.
