FFY12 Network Semi-Annual Progress Report

The FFY12 Semi-Annual Progress Report (SAPR) documents are below. The Semi-Annual Progress Report Form is intended for reporting on your Scope of Work activities encompassing the first six months of Federal Fiscal Year 2012 (October 1, 2011 – March 31, 2012). The Activity Tracking Form and supplemental SurveyMonkey are due in conjunction with the SAPR.

Webinar trainings to review how to submit the SAPR are scheduled for March 12, 2012 from 9:30 am - 11:00 am and March 14, 2012 from 1:30 pm - 3:00 pm. Registration is required and can be done here: https://www.surveymonkey.com/s/XYRQ8P2.

The Semi-Annual Progress Report documents and online SurveyMonkey are to be completed and submitted by Monday, April 16, 2012.

Please contact your assigned Network Program Manager with any questions or to request an extension on your SAPR.
