FFY 2012 Local Incentive and Nonprofit Incentive Award Renewal Process

The FFY 2012 Network for a Healthy California SNAP-Ed Plan renewal process has been simplified to an e-mail submittal of required forms to your assigned Contract and Program Managers (CM, PM).  Please refer to the Renewal Letter for important information about the renewal process.  Below is a list of required forms and attachments to assist you in completing your renewal package.  Please contact your CM and/or PM, if you have any questions.
