Local Health Department Information

Beginning in Federal Fiscal Year (FFY) 2013, the California Department of Public Health (CDPH), Nutrition Education and Obesity Prevention Branch (NEOPB) awarded funding to local health departments (LHD) to implement comprehensive Supplemental Nutrition Assistance Program - Education (SNAP-Ed) programs where LHDs serve as the lead health agency in their respective jurisdictions.

Funding is from the United States Department of Agriculture (USDA), SNAP-Ed, and continued availability of federal funding to CDPH through an interagency agreement with the California Department of Social Services (CDSS).  Grant agreements consist of a four-year term from October 1, 2012 through September 30, 2016.

Overarching Goals for the LHDs:

  • Identify and address the needs of the SNAP-Ed diverse target population within its jurisdiction;
  • Provide a comprehensive, multi-level, evidence-based, public health approach for serving their jurisdictions by directing local services and connecting with regional and statewide Network campaigns;
  • Focus activities on all levels of the social ecological model to support behavioral change;
  • Institutionalize NEOPB programs into existing social and health delivery systems; and
  • Build the capacity of low-income communities and partner agencies to provide NEOPB interventions.

Scope of Work

LHDs are required to complete specified objectives that cover a range of interventions, sectors, and activities based on their funding level for each FFY and local priorities.  The LHD Scope of Work (SOW) contains 17 objectives.  Objectives 1 through 9 are required for all LHDs.  Objectives 10 through 17 are optional/elective and are based upon individual LHD funding levels.  More information on the LHD SOW and available resources can be found on the LHD Scope of Work web page.
