Plan Submission Instructions
These instructions are applicable to all FFY 2015 SNAP-Ed funded Contractors. NEOPB requires detailed documentation from each Contractor in order to apply for the United States Department of Agriculture’s (USDA) annual State Plan. These instructions only pertain to the documents required for the State Plan submission.
Completed submissions must be received by Friday, May 9, 2014 to NEOPBfiscalrequest@cdph.ca.gov. You will receive an email receipt of your submission. If your submission is incomplete, you will be notified of any outstanding documents and your submission will not be reviewed until it is complete. The Plan Submission Checklist (Form 1) outlines the required forms to be submitted and this form must also be included with your submission.
