Network for a Healthy California (Network) FFY10 Funding Application Packet for Continuing Contractors  

In order to prepare for the FFY 2010 Plan for the United States Department of Agriculture (USDA), the Network requires detailed documentation on each continuing contractor including Local Incentive Award, Nonprofit Incentive Award, Regional Network, and Special Project contracts.  The funding application packet (FAP) for continuing contractors (those with an existing CDPH contract) has been simplified to a documentation review and teleconference with your assigned Contract Manager (CM) and Program Manager (PM).  Continuing contractors should have received most of your necessary contract documents via email on January 15, 2009 from your assigned CM or PM.  Although minimal changes are expected, the following contract documents may be necessary to complete your FAP. Please see the cover letter below for additional information.  All documents should be sent via email to your assigned CM and PM by February 5, 2009.  Please direct any questions to your Contract or Program Manager.
