California Heart Disease, Stroke, and Diabetes Prevention
In July 2013, the Chronic Disease Control Branch was awarded funding from the Centers for Disease Control and Prevention State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk factors and Promote School Health (RFA-DP 13-1305). This initiative is referenced in California as “Prevention First” and is intended to support statewide implementation of cross-cutting approaches to promote health and prevent and control chronic diseases and their risk factors, as well as to maintain coordination and collaboration across programs. This funding specifically supports the continued coordination and integration of program efforts resulting in a consolidated and combined approach redefined as the California Heart Disease, Stroke, and Diabetes Prevention.
The mission of the California Heart Disease, Stroke, and Diabetes Prevention is to reduce premature death and disability from heart disease, diabetes, and stroke.
