Service Delivery Plans
Service Delivery Plans provide a “road map” for the development of a system of care and a blueprint for the complex decisions that must be made about planning, developing, and delivering comprehensive HIV services in your communities. The HIV Care Program (HCP) requires that contractors in Direct Service Areas develop three-year Service Delivery Plans.
As outlined in Management Memorandum No. 10-02 (PDF)
, it is now time for contractors to update these Service Delivery Plans for the next contract period. Contractors are solely responsible for the completion of the Service Delivery Plans. Contractors are required to gather community input and consult with service providers when updating the Service Delivery Plans.
